I rendered this with Iray with an HDR but I get fireflies on the lips. What can I do to get rid of this?

I'm not seeing fireflies - bright white spots - on the lips in that image.
It's hard to know what we're seeing on the lips as it's not very close up for us. There's something white there, but it's hard to make out anything specific. That said, some fireflying is to be expected given the burned-out highlights elsewhere in your image. The first order of business is to make sure you don't use pure white as the glossy or reflective color for anything. Use an off-white, and adjust the roughness to prevent the strong specular highlights where the fireflies usually live.
