Script Icons, Already Existing Commands, Resetting Scripts Menu

So I noticed after an update and moveing my library to a new drive my Scripts Menu no longer has Icons or functioning Custom Actions.

I accepted the headache, and I went through and deleted the entire Scripts and Favorites Menus, applyed, saved. and restarted.

However when I try and rebuild the menu I still get the "Custom Action Already Exsists" message and there is no Icon displayed.

How to fix?

Hold on might have found the solution using the left actions window instead of the right Menus window.
