Probably the icons themselves are missing.
Have you checked the directories? 91 x91 pixels, png.
It can be a MAC thing; can't remember I've seen these on my PC.
It usually means 'Orphaned' file references in the database.
You may have had it installed to a different location, but now there may be two entries in the database, and DS doesn't know which one to display. Go to the drop-down menu in the Content Library pane, and select Content DB Maintenance, then select 'Remove Orphaned File References', see if that gets rid of the exclamation marks.

And there are eleven 91x91 .pnp files.
FixmypcMike mentioned in another thread that there was a mistake made in the metadata file for the DS Lights and Shaders (the icon was misnamed I think), so that one may not work for the moment until an update is released.
What other ones have that mark? Are you certain that you don't have these items installed into two separate Libraries?

Since I am an new hand, it will take me some time to understand how to do "Content DB Maintenance".
If there are 22 files, then 11 of them should be PNG, these are the icons for each file.
What I am saying is that I have a load of icons with the exclamation mark. They are for items that no longer exist, yet the icons remain and stubbornly refuse to be "cleaned up" by 'Remove Orphaned File References'
So I am asking if the only way is just to locate them and manually delete them?

If none of that works, and if you don't use categories, or if you do use categories, back them up first. Then use the Reset Database option, and then 're-import Metadata', that should fix it.
If you have your own categories set up, use the 'Export User Data' option FIRST, and that will be reinstalled by the Re-import Metadata option when it is run later.
In the case of the Default Lights and Shaders it's an error in the metadata -- when they split the lights and shaders out of Genesis Starter Essentials they changed the SKU # to be part of DS4, but it looks like they missed that change in the metadata so it's looking for the icon with a different file name.
Search for same file with the filesearch,rightclick on the file with "!" and "Remove Orphaned References" the other one "Cut" and move to the right position.
If you now thats the files deleted,use "Content DB Maintance" there "Remove Un-Maped Basepath and Orphans" mark the folderpath thats you know thats deleted or don't used and click "Accept".
But i use "Content Libarary "and not the "Smart Content".

(as it was the smart content window I was trying to clean up, I assumed that was where the action had to be done. But instead, I searched for the orphaned reference in the content library and found it, then did what you suggested. Worked!!)

These files were not present in the Content Library Unassigned subdir. I did Refresh anyway. I said duh a couple times. Nothing changed in the Content Library, but when I returned to Smart Content, Unassigned had straightened itself out. I don't understand it, but I'm happy with it. I'll keep it in mind for the next time.

Don't delete anything!
