That sounds like the ManifestFiles folder is losing files. Are you using the default locations for everything or have you customized them? Is it possible you're having hard drive problems?
I don't know why Spybot would find those files suspicious, unless it was a pure reputation-based analysis, but that sounds like the culprit. See if the .dsx files from the ManifestFiles folder are in Spybot's quarantine.
Also, in DIM go to Settings > Installation, right-click on the content location, and choose "Fix Installed File Registry" -- that will correct DIM's record of files that were present before DIM installed them.
If there's an update to a product (smart content added, texture tweaked, name edited, etc) then the product will go back into the DIM to be re-downloaded (But usually has a category title above them saying something like "Updated Products")

If it were a product update it would still show in the Installed tab also. Spybot still sounds like the likeliest cause.
